As student-athletes gear up for a new sports season, now is the perfect time to think about a no-cost to you sports physical at Community Health Center of Central Wyoming. Sports physicals help ensure student-athletes are healthy and ready to participate safely in school sports and activities.
Community Health of Central Wyoming will be offering sports physicals at all clinic locations this summer. Be sure to check the dates for your community below and plan ahead before practices begin.

What to Bring
Please remember to bring:
- Your insurance card
- Completed sports physical forms
- A parent or guardian signature if the student is under 18
- Glasses or contacts, if worn regularly
*We will bill Medicaid or Private Insurance for all Vaccines & Administration Fees.
